Passport Photos
Two (2) passport-size photos
Return Envelope
A large prepaid, self- addressed return envelope
Request Letter
A signed request letter (mention your current address, phone number, email and occupation)
Parents' full names, job titles, phone numbers and email addresses Family Information
Travel Confirmation
a confirmed or unconfirmed round trip ticket or letter from a travel agency
Document Legalization
If you need the embassy to legalize your documents, you must provide the following * Please note: It is recommended to provide the same document translated into French or Kirundi.
Request Letter
A signed request letter (mention your current address, phone number and email)
Notary Seal
The seal from a local notary on the document to be notarized
Immigration Status
A copy of a document stating the applicant's status in the country of resident
Money Order
A non- refundable money order/cashier's check of $40.00 (per document if different documents) payable to the Embassy of Burundi
Return Envelope
A prepaid, self-addressed return envelope
ID Required
Both the authorizer and the recipient of the Power of Attorney are required.

If you need the embassy to provide you with a life certificate, you must provide the following
