Visa Requirements

Travel Document
  • A valid travel document (minimum six month of validity before the expiration date): Passport or other travel Document.

A Completed Visa Form
  • One (1) visa application forms per applicant, legibly completed and signed.

Travel Confirmation
  • A confirmed or unconfirmed round-trip ticket or a letter from a travel agency.

Passport Photos
  • Two recent colored passport-sized photos (Please staple the photos on the forms).

Yellow Fever (YF) Vaccine
  • A copy of a yellow fever vaccine certificate.

  • A letter from the government/company.

  • An invitation letter by family members or friends.

Prepaid Return Envelope
  • A prepaid, self-addressed, stamped, express-returned envelope (such as USPS, FedEx or UPS Envelope).
